Application For Membership - Residential

I, the undersigned Applicant, am applying for membership in and agree to purchase electricity from Gascosage Electric Cooperative, Dixon, Missouri, upon the terms and conditions set forth below.

Membership: In accordance to the BYLAWS Article 1- Section 2: Joint Membership. A legally married couple may apply for a joint membership and, subject to the compliance with the requirements set forth in Section 1 of the Article, may be accepted, for such membership. The term "member" as used in these bylaws shall be deemed to include both spouses holding a joint membership and any provisions relating to the rights and liabilities of membership shall apply equally with respect to the holders of a joint membership. Without limiting the generality of the foregoing, the effect of the hereinafter specified actions by or in respect of the holders of a joint membership shall be as follows:

  1. The presence at a meeting of either or both shall be regarded as the presence of one member and shall have the effect of revoking a proxy executed by either or both and of constituting a joint waiver of notice of the meeting;
  2. The vote of either separately or both jointly shall constitute one joint vote;
  3. A proxy executed by either or both shall constitute one joint proxy;
  4. A waiver of notice signed by either or both shall constitute a joint waiver;
  5. Notice to either shall constitute notice to both;
  6. Expulsion of either shall terminate the joint membership;
  7. Withdrawal of either shall terminate the joint membership;
  8. Either but not both may be elected or appointed as an officer or director, provided that both meet the qualifications for such office;

Each residential unit and/or single family dwelling shall have installed a separate electric meter for which a separate application, membership fee and deposit shall be submitted to the Cooperative. The amount of any deposit shall be determined as follows:

  1. New residential service applicants who pose a slight credit risk (Green Light returned on the ONLINE Utility Exchange) will be charged $50.00.
  2. New residential service applicants who pose an intermediate credit risk (Yellow Light returned on the ONLINE Utility Exchange) will be charged $200.00.
  3. New residential service applicants who pose a severe credit risk (Red Light returned on the ONLINE Utility Exchange) will be charged $300.00.
  4. Notwithstanding the foregoing, if an applicant has no established credit history as indicated by ONLINE Utility Exchange, the Cooperative may establish the required deposit amount by examining past utility payment history of the applicant with the Cooperative or other utilities. Requests for voluntary disconnection of service by the applicant shall not be considered in establishing the required deposit amount.
  5. Both individuals applying for new residential service under a joint membership shall have their credit history investigated by ONLINE Utility Exchange. The amount of the deposit shall be determined by reference to the individual posing the highest credit risk according to the report of ONLINE Utility Exchange.
  6. Existing members who request an additional meter(s) shall not, in connection with establishing the amount of deposit for the new meter, have their credit history investigated by ONLINE Utility Exchange. Instead, the Cooperative may establish the required deposit amount by examining the past payment history of the applicant with the Cooperative or other Utilities.

The following are terms and conditions agreed to by the Applicant when applying for membership in Gascosage Electric Cooperative
  1. The Applicant will pay the Cooperative $__________ . If this application is accepted by the Cooperative, $25.00 of this amount shall constitute applicant's membership fee. The balance, if any, shall constitute a meter deposit, both of which shall be security for any amount owed the Cooperative by the applicant. If the application for membership is not accepted, the entire amount will be refunded.
  2. The Applicant certifies that the predominate use of electricity is residential.
  3. The Applicant will, when electric energy becomes available, purchase from the Cooperative all electric energy purchased for use on the premises described above, at rates which will be established by the Board of Directors of the Cooperative.
  4. The Applicant agrees to pay the Cooperative all amounts when due. Furthermore, Applicant hereby agrees to be personally responsible and liable for any and all expenses, including but not limited to, collection agency fees, reasonable attorneys' fees and court costs incurred by the Cooperative in the event that the Cooperative is forced to either report this account to a collection agency or pursue a civil suit In the Associate or Circuit Courts of the State of Missouri to recover an outstanding balance or debt owed to the Cooperative.
  5. If the Applicant, or Co-Applicant, owes the Cooperative from previous service and the amount is not paid in full at the time of this application, membership will be denied or revoked and service disconnected.
  6. The Applicant will cause his/her premises to be wired and maintained in accordance with the National Electricity Safety Code or other such Codes as approved by the Cooperative.
  7. The Applicant agrees to the following: "I as a condition of membership in the Cooperative, hereby grant to the Cooperative a license to enter my property for the purposes and activities set for in this paragraph and in addition will upon request grant to the Cooperative, perpetual easements without charge on and through my property to place, replace, construct, reconstruct, operate, repair, maintain and relocate either above ground or underground or a combination of both, electric distribution lines, cables, fiber optics, or other lines to provide myself and other members services from the Cooperative. Such license or easement shall also include the right to ingress and egress to, from and over the property for doing anything necessary or useful which includes the cutting or trimming of trees and shrubbery and chemical spraying of same to extent necessary to keep them clear of the line or system, cutting down all dead, weak, or leaning trees that are tall enough upon falling to strike the line or system, and to remove all structures that might endanger the line or system. I understand that the extension of service to my property will be made upon the terms and conditions set forth in the Cooperative's policies then in effect."
  8. The Applicant agrees to be bound by the provisions of the articles of incorporation and bylaws of the Cooperative, and as they may be amended from time to time and such rules, policies and regulations as may from time to time be adopted by the Cooperative.
  9. The Applicant hereby authorizes the Cooperative's Board of Directors to subscribe to "Rural Missouri" on Applicant's behalf and authorize the subscription price thereof to be deducted from any funds accruing in my favor, so as to reduce such funds in the same manner as would any other expense of the Cooperative. Said deduction not to exceed $10.00 annually.
  10. It is understood that it will be Applicant's responsibility to contact Gascosage Electric when service is to be disconnected, and to provide the Cooperative a forwarding address for final billing.
  11. The Applicant hereby acknowledges the Cooperative's unconditional right and option to require cash payment and/or money order payments from Applicant. The Cooperative's option in this regard is wholly discretionary and may vary from Applicant to Applicant based upon payment and credit history.
  12. The Applicant consents to the jurisdiction and venue of the Associate and Circuit Courts of the State of Missouri for any legal action the Cooperative is forced to commence arising from the Cooperative's provision of electric service pursuant to this Application, including, but not limited to the collection of past due account balances.
  13. The Applicant hereby agrees to, upon request, provide the Cooperative with a valid photographic identification (military, driver's license, etc.).
  14. By signing the Application, Member hereby understands, expressly consents to and agrees that the Cooperative or its agents may, at its discretion, use and valid e-mail address(es), landline telephone number(s) and/or cellular phone number(s) disclosed to the Cooperative for communicating with Member and for delivering electronic notifications to Member through the use of an automated telephone dialing system, text messaging, and/or other communication systems.
